Note:
- Print out the form and complete it. It must be signed by the applicant.
- Color Photo of the applicant must be attached.
- If you are introduced by a Member or Fellow of SNAMES. Please ask him/her to sign the 1st Proposer column on Page 2 of the application form.*
- Attach a copy of your Certificates or relavent documentation as required.
- Submit the application and the attached documents with payment to our secretariat.
* You will require two proposers, if one is already a Member or Fellow of SNAMES, our
secretariat will arrange the other for you if you cannot fine the 2nd proposer.
